Palliative care physicians
who specialize in medical care focused on pain and managing symptoms, to provide home visits as needed and work closely with the nurse case manager.
Palliative care nurse practitioner
to provide home visits as needed for expert consultation and education on the disease process and symptom management.
Registered nurse case managers
to supervise and coordinate care under the direction of our palliative care physicians.
Medical social workers
to provide therapeutic and support counseling related to the impact of dealing with an advanced progressive illness. Social workers will also assist with referrals to community resources if needed.
Pastoral care counselors
to provide spiritual support and guidance to your loved one, your family, and other caregivers.
Home health aides
to assist you with personal care, hygiene and activities of daily living.
